Who will speak for me?

Woman in HospitalChoosing someone to make healthcare treatment decisions for you when you cannot make them for yourself is probably the most important part of advance care planning.  This person is called your healthcare surrogate. 

Many people name their spouse, partner, relative, or close friend as their surrogate.  Choose one trusted person who knows you and your values.  This person need not agree with your wishes, but must respect and honor them.  It is never a good idea to appoint more than one surrogate but you can appoint an alternate surrogate in case your first choice is unable or unwilling to perform the duties.  It's best to choose your surrogate when you are well.

You may want to consider the following when choosing your healthcare surrogate:

  • How far away does this person live?
  • How easy is it to get in touch with this person?
  • Are they willing to take on this responsibility?
