Tracy Christner is the Executive Director of Project GRACE (Guidelines for Resuscitation And Care at End-of-life), an affiliate of the Suncoast Hospice in Clearwater, Florida. She is a Certified Respecting Choices® Advance Care Planning Instructor receiving her training at Gundersen Lutheran Health System in La Crosse, Wisconsin.
Tracy
provides professional and community education and programs about
healthcare decision-making in addition to providing individual and
family advance care planning services. She facilitates the Advance
Care Planning Community Partnership, a consortium of nurses,
chaplains, social workers and other professionals and has recently
started assisting with the efforts of the Florida POLST
initiative.
Passionately focused on removing barriers, Tracy is working to address disparities in the use of advance directives in the African American and Hispanic communities.
She graduated with a B.S. degree in Management from Nova Southeastern University.
Project GRACE is a nonprofit advance care planning education program that helps individuals and professionals create, communicate and honor future medical care wishes. Founded in 1999, Project GRACE's mission is to educate the community about effective advance care planning, empower individuals to communicate and implement their medical treat¬ment decisions and promote the belief that all persons are entitled to dignified care in the final phase of life.
